This screenshot shows the Create Application page. There is an option to select page type. The page types are:
Blank, List View, Column Toggle Report, Reflow Report, List View and Form, Form, and Chart. Column Toggle Report page type icon is selected.

Other options in the page are:
Action: Add a column toggle report
Parent Page:Deafault is selected
Pge Source: Table
Table Name: Employees is selected

There are two buttons:
Cancel
Add Page. Add Page is focused.
